We are a community-driven healthcare organization dedicated to serving our patients and their families locally, in the neighborhoods that they live and trust. We provide quality and compassionate healthcare services to many diverse communities and do so in a way that consistently overcomes economic, cultural, and geographic barriers.

Our strength is in our constant focus on being a community partner that cares for the whole person. We view our patients as being individuals that deserve personalized care. Each patient has a very unique need, and we strive to develop programs that not only meet these needs but also exceed the expectations of those receiving care.

Locally, we are one of our region’s largest not-for-profit providers of health and wellness services, caring for nearly 70,000 patients annually and contributing more than $87.9 million in economic impact on our region.

We are a Federally Qualified Health Center (FQHC) organization that began its operations in 1972 in Apopka, Florida. We have voluntarily chosen to be accredited by the prestigious Accreditation Association for Ambulatory Health Care Inc. and are covered under the Federal Tort Claims Act (FTCA). This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). Community Health Centers is a 501c3 nonprofit, registered in the US, EIN 59-1480970.

Beyond our healthcare services, we strive to be an active, visible and engaged member of the communities we serve. We proudly embrace the responsibility of being an active leader in improving the overall wellness of our community.

Our board and team members participate in numerous programs and events to give back to those we serve. Whether it be volunteering at a local food pantry, helping build a home for Habitat for Humanity, or taking part in a community parade to recognize and celebrate Martin Luther King Jr., we proudly share in the excitement and importance of these events.

By having a strong community focus, a non-profit organization such as ours is able to develop a strong community reputation. By building trust with our patients, we are able to truly become partners in not only their care but also their life. We seek to care for the whole person, and this includes assisting with nutrition and emotional health as well as physical wellness.
