Much like a home, our goal is to make it easy and comfortable for you to get the care you need in a way that works best for you. Within our home, a relationship develops between you and your Care Team: the providers, clinical support, and office staff dedicated to coordinating the services you need for the best care possible. How Do I Become a Patient?

Becoming a patient at Community Health Centers is easy, and anyone can join. Start taking advantage of our affordable health services and the convenience offered with over a dozen locations throughout central Florida. 

STEP 1

Request Appointment

Schedule your first appointment by calling us at 407-905-8827 (or 352-314-7400) or using our online appointment request form.

STEP 2

Patient Intake Forms

Complete our online Patient Intake form, or it can be completed in person on the day of your scheduled appointment.

STEP 3

Visit Doctor

After completing your Patient Intake Forms, the only thing left to do is attend your scheduled appointment.

How Do I Pay For Services?

We accept most types of insurance plans, including Medicare, Medicaid, HMOs, PPOs, KidCare, Medikids, Healthy Kids, Children’s Medical Services (CMS), and other plans. Certified Application Counselors are available to assist in finding and enrolling in insurance options through the marketplace.

It is our vision to provide essential medical services regardless of immigration status or inability to pay

Items to Bring to Your First Visit

  1. Photo identification
  2. Health insurance card(s)
  3. Social Security card
  4. Proof of income for Sliding Discount Program
  5. Medical or dental history
  6. Medical Records – especially if you’ve recently been admitted to the hospital or ER
  7. All medications currently taking
  8. Foster parent or grandparent: Guardian or custody papers showing custody of child required
  9. Adult guardian: Power of Attorney

Access Availability

We offer same-day access at limited locations. Contact us at (407) 905-8827 to make an appointment or walk-in to your participating center. For faster service please bring a photo ID, insurance card (if applicable), copay, list of medications you’re currently taking, and any medical records that you may have.

After Hours

Providers are available for after-hours emergency calls through our answering service at 407-905-8827. In the event of an emergency, please call 911.

Community Health Centers is an accredited medical and dental home with the Accreditation Association for Ambulatory Health Care.
