Requesting an Appointment
Requesting an appointment has never been more convenient. Patients can contact us by phone to speak directly with a scheduling specialist, or submit an online request, to have a scheduling specialist contact you. We do offer same-day access, and walk-in appointments, at limited locations.
Appointments can be requested online at any time. A scheduling representative will reach out to you by phone to confirm your request.
To protect your privacy, please refrain from providing personal health information or any other sensitive information via contact form. A patient service rep will contact you and request the needed information.

Monday: 7am – 6pm
Tuesday: 7am – 7pm
Wednesday: 7am – 5pm
Thursday: 7am – 5pm
Friday: 7am – 1pm
All patients should bring a photo ID, insurance card (if applicable), copay, list of medications you’re currently taking, and any medical records that you may have. New patients may be asked for additional items to bring at the time of their appointment.
Changing an Appointment
Appointments can only be rescheduled or canceled by calling us at 407-905-8827 or 352-314-7400.
